MARTIAL ARTS WAIVER AND RELEASE

I, the undersigned ("Client"), understand and acknowledge that by engaging in martial arts training at the The Void MMA will subject you to extensive physical exercise and training programs that carries inherent risks and that the services provided by The Void MMA, involve potential exposure to injury or other risks. By signing this waiver and release form, I voluntarily assume all risks related to my participation in the martial arts training sessions and any associated activities.

Assumption of Risk: I am aware that the activities involved in martial arts training sessions may include, but are not limited to, cardiovascular exercise, strength training, flexibility exercises, and other fitness-related activities. I understand that these activities involve the risk of injury, including but not limited to muscle strains, sprains, broken bones, cardiovascular complications, and even death. I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, that I might incur as a result of participating in the personal training sessions.

Medical Clearance: I hereby certify that I have consulted with a qualified healthcare professional regarding my physical condition and any pre-existing medical conditions before beginning the martial arts training sessions. I confirm that I am physically able to engage in the activities associated with the personal training sessions and that I am not aware of any medical reasons that would prevent me from participating.

Release of Liability: In consideration of the services provided by The Void MMA, I hereby waive, release, and forever discharge The Void MMA, Trainer, their affiliates, employees, and agents from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me during or after the martial arts training sessions.

Indemnification: I agree to indemnify, defend, and hold harmless The Void MMA, Trainer, their affiliates, employees, and agents from any claims, liabilities, costs, or expenses, including reasonable attorney fees, arising out of my participation in martial arts training sessions.

Photographic and Video Release: I grant permission to Void MMA to use any photographs or videos taken during the martial arts training sessions for promotional, marketing, or educational purposes without compensation.

Severability: If any provision of this waiver and release form is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I have read and fully understand the terms of this waiver and release form. I acknowledge that I am signing this agreement freely and voluntarily, without any inducement, assurance, or guarantee being made to me. I intend for this waiver and release to be binding upon my heirs, executors, administrators, and assigns.
